目的：探讨切取鼻中隔软骨和筛骨垂直板并且联合运用耳软骨矫正短鼻畸形的临床可行性。

方法：2019年11月至2021年8月，24例先天性短鼻畸形患者在我院行鼻整形手术。所有患者均切取鼻中隔软骨，筛骨垂直板以及部分耳廓软骨作为鼻中隔延伸移植物重塑鼻尖形态。术中获取鼻中隔软骨平均面积为(19×15)mm²，筛骨垂直板骨片平均面积为(25×10)mm²，鼻中隔保留12mm宽度的“L”形支架。鼻背植入膨体聚四氟乙烯增加其高度。术后通过数字化技术分析图像软件对手术前后鼻外观形态进行比较，以评价手术效果。

结果：24例患者均顺利完成手术，切口处皮瓣血运良好，未发生感染血肿等并发症。经过7～9月随访，术后无软骨和假体外露，亦无鼻中隔穿孔等不良并发症的出现。2例患者诉嗅觉减退，随访观察3个月后自行恢复。术后未出现鼻通气不畅及其他功能障碍发生。手术前后鼻长和鼻尖突出度的差别有统计学意义（P<0.05），鼻尖旋转度的差别无统计学意义（P>0.05）；手术前后鼻尖阻力的差别有统计学意义（P<0.05）。

结论：筛骨垂直板辅助鼻中隔软骨和耳廓软骨可以有效增加鼻长和鼻尖突出度，可明显改善短鼻形态外观，但对鼻尖旋转度的改变有限。

Objective: To investigate the experience and clinical effect of septal cartilage and ethmoid vertical plate combined with ear cartilage in the correction of short nose deformity.

Methods: From November 2019 to August 2021,24 patients with congenital short-nose deformity underwent rhinoplasty in our hospital. Septal cartilage, vertical plate of ethmoid bone and part of auricle cartilage were removed from all patients as septal extension graft to reshape nasal tip shape. The average area of the septum cartilage obtained during the operation was （19×15）mm², and the average area of the vertical plate of the ethmoid bone was （25×10）mm². The "L" shaped stent with a width of 12mm was retained in the septum. Enlarged Polytetrafluoroethylenes was implanted on the nasal dorsum to increase its height. After surgery, the nasal appearance was compared with that before and after surgery by digital image analysis software to evaluate the surgical effect.

Results: All the 24 patients completed the operation successfully, with good blood flow of the flap at the incision and no complications such as infection and hematoma. After 7-9 months of follow-up, there was no cartilage and prosthesis exposed, and no nasal septum perforation and other adverse complications. Two patients complained of hyposmia and recovered spontaneously after 3 months of follow-up observation. No nasal obstruction or other functional disorders occurred after operation. There were statistically significant differences in nasal length and nasal tip protrusion before and after surgery (P<0.05), but no statistically significant differences in nasal tip rotation (P>0.05). The difference of nasal tip resistance before and after operation was statistically significant (P<0.05).

Conclusions: The vertical plate of ethmoid can effectively increase the nasal septum cartilage and auricle cartilage, and can significantly improve the appearance of short nose, but the change of nasal tip rotation is limited.
