背景:Ilizarov技术作为一种结合了“牵张成骨”原理的外固定治疗技术,自上世纪50年代由苏联骨科专家Iizarov教授提出以来,在全世界获得了广泛的应用。如今,Ilizarov技术可在一些疑难疾病治疗方面起到特殊的作用,被一些学者认为是骨缺损、慢性骨感染的治疗“金标准”。然而,目前尚未有文献使用定量和可视化的方法对此领域的研究状况进行全面的总结,文献计量学技术作为一种专业的文献情报分析技术,可用于探索Ilizarov技术的相关文献情报,描绘Ilizarov技术的研究历史脉络。

目的:本研究致力于使用文献计量学的技术方法对国内外Ilizarov技术的相关文献进行可视化计量分析,探讨该领域的研究现状、研究热点和发展趋势。

方法:中文文献以Ilizarov等相关关键词检索中国知网数据库、万方数据库、维普数据库、中国生物医学文献服务系统(SinoMed),英文文献以Ilizarov method等相关关键词检索Web of Science(WOS)核心合集数据库,检索2021-01-05之前所有与Ilizarov技术相关的文献,并提取作者、期刊、国家、机构、关键词、被引率等数据,用文献计量学方法对来源数据进行统计分析的同时,利用VOSviewer等软件绘制可视化知识图谱。同时,使用知网中国引文数据库和WOS数据库检索2012-2022年间被引频次排名前100的中英文文献。

结果:共检索出中文文献1 789篇、英文文献1 709篇。2020年中国Ilizarov技术相关SCI文献年发表量居全球第一。发表英文文献总量排名前三的国家分别为美国、英国和中国。中文文献载文量排名前5的期刊为中国矫形外科杂志、中华创伤骨科杂志、中国修复重建外科杂志、中华骨科杂志及中国骨与关节损伤杂志;发表英文文献数量排名前7的核心期刊中,JCR分区骨科Q1区的期刊有Clinical Orthopaedics and Related Research、Journal of Bone and Joint Surgery - British Volume、 International Orthopaedics。VOSviewer的可视化关键词聚类分析将中文文献中的研究方向分为6类,其中相对较新的研究热点是感染性骨不连的治疗以及糖尿病足的治疗,英文文献中的研究方向可分为4类,相对较新的研究热点是感染及创伤后骨缺损的修复。

结论:①尽管Ilizarov技术已经有70余年的研究应用历史,全球对Ilizarov技术的研究产出速度仍在增长,来自中国的研究逐渐成为Ilizarov技术相关研究中的重要组成部分,但国内学者尚缺乏国际合作。②在中国,Ilizarov技术的疗效逐渐得到证实,适用范围得到进一步拓展。近10年里,骨缺损、骨不连、骨髓炎、四肢畸形的治疗,以及糖尿病足治疗与微循环重建是国内学者研究的重点。③Taylor外固定架技术、混合固定技术、膜诱导技术对Ilizarov技术的部分并发症起到了改良作用。

Objective: To analyze the literature of Ilizarov methods, visualizing and discussing the research status, research hotspots and research trends.

Methods: A bibliometric study on Ilizarov methods was performed. Using keyword “Ilizarov” and the Chinese translations to retrieve Chinese publications from CNKI database, WANFANG DATA, VIP database and SinoMed database. Using keywords “Ilizarov method, Ilizarov technique, Ilizarov treatment” to retrieve English publications from Web of Science Core Collection database before 2021-01-05, extracting the information including author, journal, country, institution, keywords and cited times. Using the tools of bibliometrics and VOSviewer to analyze the data and draw knowledge mapping.

Results: A total of 1 789 Chinese publications and 1 709 English publications were included. USA, UK and China were on the top 3 list of the number of publications. Orthopaedic Journal of China, Chinese Journal of Orthopaedic Trauma, Chinese Journal of Reparative and Reconstructive Surgery, Chinese Journal of Orthopaedics, and Chinese Journal of Bone and Joint Surgery published the most literature. 7 English journals were identified as core journals in the field of Ilizarov methods, among which Clinical Orthopaedics and Related Research, Journal of Bone and Joint Surgery - British Volume, International Orthopaedics were JCR Q1 journals. According to the visualized keywords co-occurrence cluster analysis, the research topics in Chinese literature could be sorted into 6 clusters, the latest hotspots were bone nonunion caused by infection and the treatment for the diabetic foot. In the English literature, the research topics could be sorted into 4 clusters, while the latest topics were infected and posttraumatic bone defects.

Conclusion: ①Although Ilizarov method has been applied for more than 70 years, the publication of research is still increasing worldwide. China has gradually become an important research force of Ilizarov method. ②In China, the clinical efficacy of Ilizarov method has been confirmed, and the application of Ilizarov method has been further expanded. In recent 10 years, the treatment of bone defect, bone nonunion, osteomyelitis, limb deformity, diabetic foot treatment and microcirculation reconstruction were the focus of domestic researchers. ③Taylor spatial frame, hybrid fixation method and Masquelet technique reduced some of the complications of Ilizarov method.
