世界卫生组织提出“到2030年终结艾滋病流行”的战略目标。然而,抗逆转录病毒疗法在终结艾滋病流行方面的局限性愈发明显,开发有效的治愈性干预措施现已成为生物医学领域的重要课题。HIV治愈的研究的目标包括完全治愈和功能性治愈,分别指彻底根除体内所有具有复制能力的HIV颗粒和持续维持低于标准测定定量水平的病毒载量。随着科学进步和发展,伴随而来的是伦理问题的日益复杂和深化。然而,目前国内对于该领域的伦理问题探讨较少,需要系统、深入地论证分析,并依据我国实际情况进行伦理视角的思考。本研究综合运用文献研究法、伦理论证法和访谈法,提出HIV治愈研究的伦理评价框架,分析存在的主要伦理问题并给出策略建议。 后果论和义务论是评价HIV治愈研究的理论基础,在此基础上综合应用受益原则、尊重原则、公正原则以及负责任的研究与创新原则,建立完整全面、动态平衡的伦理学评价框架。通过对HIV治愈研究的受益分析,发现其存在独特优势。在个体层面上包括心理和情感满足、健康教育和信息获取以及减轻治疗负担,在公共卫生层面上包括降低传播风险和长远的社会经济效益,同时也能够促进科学进步。本研究发现了目前HIV治愈研究,尤其是HIV治愈相关临床试验中存在的主要伦理问题,包括受益与风险平衡问题、知情同意问题、群体代表性与公平性问题、污名化和隐私保护问题以及伦理审查有效实施问题。本研究针对性地提出了应对HIV治愈研究中伦理问题的建议,包括以下五个方面:完善受益风险评估和管理、强化知情同意规范性和有效性、提升群体代表性与公平性、加强隐私保护和权益保障以及改进伦理审查与监管机制。 |

The World Health Organization has proposed the strategic goal of "end the AIDS epidemic by 2030." However, the limitations of antiretroviral therapy in ending the AIDS epidemic have become increasingly apparent, and the development of effective curative interventions has now become an important topic in the field of biomedicine. The goals of HIV cure research include sterilizing cure and functional cure, which respectively refer to the complete eradication of all replicable HIV particles in the body and the sustained maintenance of viral load below the threshold of standard quantification. With the development of scientific, the ethical issues accompanying this progress have become increasingly complex and profound. However, there are few discussions on ethical issues in this field domestically, which need systematic and in-depth demonstration and analysis, and ethical thinking according to the actual situation in our country.This study uses literature review, ethical argumentation, and interview methods to propose an ethical evaluation framework for HIV cure research, analyze the main ethical issues involved and provide strategic recommendations. Utilitarianism and deontology are the theoretical foundations for evaluating HIV cure research. On this basis, the principles of beneficence, respect, justice, and responsible research and innovation are comprehensively applied to to establish a complete, comprehensive, and dynamically balanced ethical evaluation framework. Through the benefit analysis of HIV cure research, this study identifies its unique advantages. At the individual level, it includes psychological and emotional satisfaction, health education and information access, as well as reducing the burden of treatment. At the public health level, it includes reducing the risk of transmission and long-term socioeconomic benefits. At the same time, it can also promote scientific progress. This study identified the main ethical issues existing in current HIV cure research, particularly in HIV cure-related clinical trials, including issues of balancing benefits and risks, informed consent, population representation and fairness, stigmatization and privacy protection, and effective implementation of ethical review.This study puts forward targeted recommendations for addressing ethical issues in HIV cure research, which include the following five aspects: improving benefit-risk assessment and management, enhancing the standardization and effectiveness of informed consent, improving population representation and fairness, strengthening privacy protection and rights safeguarding, and improving ethical review and regulatory mechanisms. |
