摘要 研究目的 1. 主要目标 本项目通过对妇幼医务工作者、女性妇联工作者(统称宫颈癌筛查相关工作者)进行问卷调查,获取工作者参与宫颈癌筛查工作积极性的影响因素,提出针对性的干预措施和政策建议,为人员的管理与培训提供参考。 2. 次要目标 通过对初筛异常妇女开展宫颈癌筛查服务满意度及需求优先级的问卷调查,明确该人群对筛查服务的评价核心维度与未满足需求,从而为妇幼保健机构优化筛查服务体系、提升服务质量提供依据,以期实现筛查满意度和接受率的提升。
研究方法 妇幼医务工作者参与宫颈癌筛查工作积极性及知识水平的影响因素分析 采用方便抽样的调查方法,调查鄂尔多斯市和山西武乡妇幼保健院医务工作者宫颈癌知识水平和参与相关筛查工作的积极性。问卷分为一般人口学特征、宫颈癌知识、工作意愿3部分,采用logistic分析工作者参与筛查工作积极性及知识水平的影响因素。 鄂尔多斯市女性妇联工作者参与“两癌”筛查工作积极性影响因素分析 采用方便抽样的调查方法,调查鄂尔多斯市女性妇联组织工作者关于妇女“两癌”知识水平和参与相关筛查工作的积极性。由工作人员通过妇联组织工作群转发等方式,邀请妇联工作者填写问卷。问卷分为一般人口学特征、“两癌”知识、工作意愿3部分,采用logistic回归分析其工作积极性影响因素。 山西武乡初筛异常随访妇女宫颈癌筛查满意度影响因素分析 采用方便抽样的调查方法,调查山西长治市武乡县2024年度宫颈癌初筛异常妇女对宫颈癌筛查的满意度评价。问卷分为一般人口学特征、宫颈癌知识、筛查满意度评价3部分,采用logistic分析妇女对筛查满意度的影响因素,并且使用Kano模型分析妇女对具体筛查服务的偏好。
研究结果 共收集833份有效妇幼保健院工作者问卷,工作者总体知识水平和工作积极性较高。关于宫颈癌知识水平的多因素分析结果显示:少数民族宫颈癌知识水平较低;女性医务人员知识水平较高;具备中级职称者知识水平较高;宫颈癌筛查工作者知识水平较高;接受“两癌”健康教育者知识水平较高(P<0.05)。关于工作意愿的多因素logistic分析显示:对重点筛查人群知晓率更高的医务人员积极性更高(P<0.05)。 共收集492份妇联部分有效问卷,工作者总体知识水平和工作积极性较好,但仍有个别知识掌握不足,工作积极性亦有提升空间。多因素回归分析结果显示,工作地区为伊金霍洛旗,工作年限为6~10年,研究生及以上学历,“两癌”知识水平高的工作者工作积极性更高(P<0.05)。 共收集143份妇女满意度有效问卷,妇女整体满意度较高,最终满意度为4.65±0.32分(满分5.00分)。58人比较满意(最终满意度≥4.00分且<4.50分),80人非常满意(最终满意度≥4.50分),总满意率为96.50%。多因素分析结果显示:宫颈癌知识水平高、从事工商服务业的妇女满意度更高;BMI过低、超重、肥胖,有糖尿病、高血压等基础病,有宫颈炎或阴道炎的妇女满意度偏低。
研究结论 少数民族、无职称、以及男性妇幼保健院医务工作者知识水平有待提高;对重点筛查人群知晓率更高的医务工作者参与筛查工作积极性更高,应加强此方面健康教育与培训。 妇联工作者对“两癌”核心知识尤其是HPV无症状感染等关键信息的掌握不足。未来需要通过健康教育与培训提高妇联工作者知识水平,以期促进其积极参与妇女“两癌”筛查工作。 妇女宫颈癌筛查服务整体满意度接近100.00%。BMI偏低或偏高、患基础病、妇科病的群体满意度偏低,需针对性改善服务体验。数字化健康管理和无痛检查体验是超越患者预期的“增值服务”,但居家自检服务接受度不及预期。建议未来加强健康宣教,推广智能健康管理系统,优化检查舒适度,以多维度改进策略提升宫颈癌筛查满意度。 |

Research Objectives 1. Primary Objectives This research employs questionnaire surveys among maternal and child healthcare workers and female Women's Federation staff (collectively referred to as the cervical cancer screening workforce) to identify factors influencing their engagement in screening work. Based on the findings, we propose targeted intervention strategies and evidence-based policy recommendations, providing actionable insights for optimizing personnel management and training programs. 2. Secondary objectives To obtain women's satisfaction status with cervical cancer screening services through the cervical cancer screen satisfaction questionnaire and the service questionnaire, so as to provide references for improving the level of medical services in maternal and child health hospitals,and improving women's satisfaction, increasing the acceptance rate of screening.
Research Methods 1. Analysis of factors influencing the motivation and knowledge level of maternal and child medical workers' participation in cervical cancer screening work In June-July 2023 and January 2025, respectively, the survey method of convenience sampling was used to investigate the knowledge level of cervical cancer and the motivation of medical workers to participate in related screening work in Ordos and Shanxi Wuxiang Maternal and Child Health Hospital. The questionnaire was divided into 3 parts: general demographic characteristics, knowledge of cervical cancer, and willingness to work . Logistic was used to analyze the influencing factors of workers' motivation to participate in screening work and their knowledge level. 2.Analysis of factors influencing the motivation of female women's federation workers to participate in “two cancers” screening work in Ordos. In June-July 2023, the survey method of convenience sampling was used to investigate the knowledge level of women's “two cancers” and the motivation of women's federation workers in Ordos City to participate in the relevant screening work. The workers were invited to fill in the questionnaire by the staff through the work group of women's federations and other ways. The questionnaire was divided into three parts: general demographic characteristics, knowledge of the two cancers, and willingness to work. Logistic analysis was used to analyze the factors affecting the motivation of female workers to participate in the two cancers screening. 3. Analysis of factors influencing the satisfaction of cervical cancer screening among women with abnormal initial screening and follow-up in Wuxiang, Shanxi Province. From September 2024 to February 2025, a convenience sampling method was used to investigate the satisfaction evaluation of cervical cancer screening among women with abnormal primary screening for cervical cancer in Wuxiang County, Changzhi City, Shanxi Province in the year 2024. The questionnaire was divided into 3 parts: general demographic characteristics, knowledge of cervical cancer, and evaluation of satisfaction with screening, and logistic was used to analyze the factors influencing women's satisfaction with screening, and Kano model was used to analyze women's preferences for specific screening services.
Results (1) A total of 833 valid questionnaires were collected from workers of maternal and child health centres, and the overall knowledge level and work motivation of the workers were high. Knowledge of cervical cancer was lower among ethnic minorities; higher among female medical workers; higher among those with intermediate titles ; and higher among those who had received health education on ‘two cancers’ ; and higher among those who received cervical cancer screening . The knowledge level of cervical cancer screening workers was higher ; and the knowledge level of those who received ‘two cancers’ health education was higher. Multifactorial logistic analysis on willingness to work showed that health workers with higher knowledge of key screeners were less likely to perceive that the screening process took up their time (P<0.05). (2) A total of 492 partially valid questionnaires were collected from the FMC, and the overall knowledge and motivation of the workers were good, but there were still individuals with insufficient knowledge and room for improvement in motivation. The results of multifactorial logistic regression analysis showed that the working area was YijinhuoLuo Banner , the working years were 6~10 years , with postgraduate education or above , and workers with postgraduate education or above and high knowledge of the ‘two cancers’ were more willing to participate in the ‘two cancers’ screening work (P<0.05). (3) The overall satisfaction of women was high, with a final satisfaction score of 4.65±0.32 out of 5.00. 58 were relatively satisfied (final satisfaction score≥4.00 and <4.50), and 80 were very satisfied (final satisfaction score ≥4.50), with an overall satisfaction rate of 96.50%. The results of multifactorial analysis showed that women with high knowledge of cervical cancer and working in business and service industry were more satisfied, and women with low BMI , overweight,obesity, having an underlying disease such as diabetes or hypertension , having cervicitis or vaginitis women had a lower level of satisfaction (P<0.05).
Conclusion (1) The knowledge of ethnic minority, non-titled, and male medical workers in maternal and child health centres needs to be improved; medical workers with a higher rate of knowledge of key screening populations are more motivated to participate in screening work, and health education and training in this area should be strengthened. (2) Women's Federation workers do not have sufficient knowledge of the core knowledge of the ‘two cancers’, especially key information such as asymptomatic HPV infection. In the future, it is necessary to improve the knowledge of women's union workers through health education and training, in order to promote their active participation in women's ‘two cancers’ screening work. (3) The overall satisfaction rate of women's cervical cancer screening service is close to 100.00%, while the satisfaction rate of groups with low or high BMI, basic diseases and gynaecological diseases is low, so it is necessary to improve the service experience. Digital health management and painless examination experience are value-added services that exceed patients' expectations, but the acceptance of home self-examination service is not as expected. It is recommended to strengthen health education, promote intelligent health management system, optimise the comfort of examination, and enhance the satisfaction of cervical cancer screening with a multi-dimensional improvement strategy. |
