目的:探讨在结肠镜检查前使用虚拟现实(virtual reality, VR)视频教育患者能否改善肠道准备质量,并提高患者对结肠镜检查的满意度。

方法:2018年10月1日至2020年11月1日符合条件的患者被随机分配至对照组或试验组,所有患者均进行清醒结肠镜检查。对照组按照传统方式(口头宣教+纸质资料)进行肠道准备的教育,试验组在传统方式的基础上增加VR视频教育。研究的主要终点是通过Boston肠道准备量表(BBPS)评估肠道准备质量。次要终点包括息肉检出率、腺瘤检出率、术前睡眠质量、患者依从性、总体满意度以及再次进行结肠镜检查的意愿。

结果:共有346名门诊患者参加了该试验,每组有173名患者 (对照组平均年龄为50.5±12.5岁,试验组平均年龄年龄为52.6±11.4岁)。两组的基线特征(人口学信息、既往病史、生活方式和排便特点等)无明显差异。VR视频组的BBPS评分显著高于对照组(7.61±1.65 vs 7.04±1.70,P=0.002);息肉检出率(41.9% vs 26.7%,P=0.003)和腺瘤检出率(32.6% vs 22.1%,P=0.03)在VR视频组也更高。接受VR教育的患者有更好的依从性(68.8%vs 50.3%,P0.001)和更高的满意度(8.68±1.70vs8.16±2.15,P=0.01)。

结论:VR视频教育能够改善结肠镜受检者的肠道准备质量,提高患者的依从性和满意度。

Objective:To investigate whether using VR videos for patient education before colonoscopy could improve bowel preparation.

Methods:Eligible patients undergoing conscious colonoscopy from October 1, 2018 to November 1, 2020 were randomly assigned to either the control group or the invention group. The control group received only conventional education on bowel preparation before colonoscopy (oral instructions and written materials that had the same contents), and the invention group received VR video and the conventional education. The primary outcome was the quality of bowel preparation measured by the Boston Bowel Preparation Scale score. Secondary outcomes included polyp and adenoma detection rates, compliance with complete bowel cleansing, preprocedural anxiety, overall satisfaction, and willingness to undergo a follow-up colonoscopy. 

Results: A total of 346 outpatients were enrolled in the trial, with 173 patients randomly assigned to each group (control group: mean age, 50.5 ±12.5 years; VR video group: mean age, 52.6±11.4 years). Baseline characteristics, including demographic information, medical history, lifestyle, and the characteristics of stool, were comparable between the VR video group and the control group. The mean Boston Bowel Preparation Scale score was significantly higher in the VR video group than in the control group (7.61±1.65 vs 7.04±1.70,P =0.002). The detection rate of polyps (41.9% vs 26.7%, P = 0.003) and the detection rate of adenomas (32.6% vs 22.1%, P = 0.03) were also higher in the VR video group. Patients who received VR education had better compliance (68.8% vs 50.3%, P < 0.001) and higher mean overall satisfaction (8.68±1.70 vs 8.16±2.15,P =0.01) with bowel preparation.

Conclusion: Patients who received VR video education before colonoscopy

had better bowel preparation, higher polyp and adenoma detection rates, and improved compliance and satisfaction.
